How Can This Happen?

I am 6.5 mo. pregnant and concerned about something that recently happened to another expectant mom with whom I'm aquainted. She hadn't felt her baby move for a couple of days and finally decided she should go and get checked out. When she went they couldn't find a heartbeat so they admitted her into the hospital. They discovered that the baby had been dead for 2 days. This woman was one week away from her expected delivery. They said the babys death was due to the umbilical cord being wrapped around its neck. My question is how can this happen? I thought the umbilical cord being wrapped around the neck only posed a threat when in labor and delivery, not while actually still in moms womb. How often does this happen? And, is there anything you can do to prevent it?
