My fiance was born without a uretha so they had to build one. Would this count for us having a hard time getting pregnant?

He might need to ask his doctor. They sell male fertility tests at the drugstore. Has he tried one yet?

Since the semen is ejaculated through the urethra, I'd say there could be a concern. However, keep in mind that it takes an average of 6-9 months for healthy coulples who are hitting the timming just right every month to become pregnant. Most doctors don't want you to see them for problems getting pregnant until after one full year of trying! However, in your case I'm sure a urology consult would be in order. best to you!
