My son had a cavity fixed when he was 4. He did just fine with it, they gave him a little gas and he was in la la land from there on, he never felt a thing. He actually said he had fun at the dentist.

I don't remember how old I was when I got my first cavity, but I have the type of teeth that are very prone to getting cavities. I don't remember when I had my first filling done, but I know I had a few before having a tooth pulled at 4 or 4 1/2. I believe it was one that had already been filled once before but the filling fell out and it was rotten the whole way through (I kept the tooth for a long time, so I remember that part). They just used gas for me for the cavities -- no needles. They probably used a few jabs for the tooth removal though.

try to see a paedodontist (for kids). They have so many techniques, and really make the kids feel safe. if he is really not co-operative, they may suggest sedation (conscious or asleep). This is a critcal age for kids...if they have a bad experience now, it could affect the way the feel about the dentist forever. Good luck!

Definitely take your child to a Pedodontist. Most adult practices aren't equipped to deal with frightened kids, and it can quickly turn in to a traumatic experience. I was a Dental Assistant for 10 years and saw this all too often.
