Maternity Leave Letter To Employer

Hi, I was wondering what information needs to be included on the Maternity Leave letter to the employer. I am about to write mine up but i want to make sure that all the info is there and that i won't miss something that will cause an problem later on. Do you have to provide a medcal certificate to confirm the pregnancy? Besides the date you want to leave and the date you are planning to return to work... is there anything else? Thanks,

I don't know. With my company my doctor needs to submit a form after birth to let them know to start the short term disability. I'm going to call my boss when I get admitted to the hospital. I don't think I have to write a formal letter but I guess I should check on that.
