Once an egg is released, a womans body releases hormones which prevent any other eggs from maturing/being released. It would be unusual but not unheard of for multiple ovulation to occur. The instances of twins being a week or two apart is easily explained - different IMPLANTATION dates. That is, when the fertilised egg attaches to the wall of the uterus and begins to develop. It is possible to have 2 fathers for the one twin pregnancy (Ever read Tommo Hawk by Bryce Courtney?)
The mucous plug is formed at about 12 weeks (Hence the reduced risk for miscarriage after this date)
