If you have a tattoo on your lower back, receiving an epidural WILL put you at risk for infection and in worst case scenario, paralysis. If you inquire about this with any credible doctor, he will tell you the same and those who do not want take such risks will not perform the epidural if you have a tattoo on your lower back.
