The first visit is mostly blood work, pap, urine test, blood pressure, and depending on how far they think you are, they might listen for the heartbeat and some will even do a vaginal ultrasound to help date the pregnancy. Things to ask would be about symptoms you are having and concerned about or need relief from. They will want to know a family history and will ask you some questions, make sure you know when your last period was. Its pretty straight foward.
